We greatly appreciate every visit you make to a peer who has experienced a suicide loss! Please be sure to complete this summary form within 48 hours of each visit you make. You will receive an automatic confirmation by email shortly after submitting the form to let you know we've received it.
If you have questions about your role or obligations as a Survivor Outreach Program volunteer, please contact your AFSP Chapter's Survivor Outreach Program Coordinator. You can view our online SOP Training Program at any time, day or night, to refresh your skills or learn new ones before making a Survivor Outreach Program visit. This training is password protected. If you need the password, please contact the Loss and Bereavement Department at email@example.com.
